Good question. I just read this as I was getting close to the Audiology clinic so I popped in there. According to one of the nurses that works with hearing aids, there's no longer a set time where you get them automatically replaced. If you think they are bad bad, not working or something like that, you go to the clinic and they will get you a new hearing test, and/or they will attempt to repair the ones you have. If they are unrepairable and the doc says you still need them then they will order you a new pair. But I can tell you if you break one like I did, I step on it and crushed it, they do replace it pretty soon.
